Individuals have the right to access and request changes to their health records under the Health Insurance Portability and Security Act (HIPAA). MyWellSpan is a convenient way for users to manage their health information and care from a computer, tablet or smartphone at any time.
To access your medical record through MyWellSpan, log into your account and go to the Sharing section. From there you can request your medical records by providing the required information. If you are unable to use the online portal, you can also complete a paper form, Authorization to Use or Release Health Information, which is available in multiple languages.
If you need to change your health record, the MyWellSpan portal provides an easy way to submit a request. Simply access the Document Center, click on Requested Records and Amendments, and fill out the Request for Amendments form. Alternatively, you can complete a patient request amendment form and mail it to WellSpan Health.
Additional forms are available for specific situations such as requesting medical records for a deceased loved one or making other types of 21st Century Cures Act requests. These forms may be used for various purposes related to accessing, sharing or using electronic health information and should be completed accordingly.
